The Database Design Proposed are the following:

1. Design a working prototype using SQLite Database. The SQLite database is a small, lightweight database application, suited for learning SQL and database concepts, or to just explore some database-related ideas without requiring a full-blown database management system (DBMS).

2. Provide a brief synopsis (utilizing research from related assignments) analyzing the detailed requirements of your prototype database design.

3. Design a database prototype that includes diagrams, data dictionary, design decisions, limitations, etc. The database should consist of at least four tables, two different user roles, and two reports.

USER ROLE:

Program coordinator= database administrator/authorize who able to access and use the database. Responsible for review result from periodic audition and the final report before submission.

Lifestyle Coach====are user able to access and upload new data (cannot delete/update). Responsible for periodic database audit.

Database development team (3 nurse) =====responsible for update, backup, and maintenance as well as retrieve the final report for submission.

I. Thoughtfully analyzes and evaluates the detail requirements. Draws warranted, judicious, nonfallacious conclusions. Information and evidence are accurate, appropriate.

II. Demonstrates a full and deep knowledge of the subject. Develops all of the database prototype assignment components. Accurately presents the impact of the database in practice.

III. Content is comprehensive, accurate, and persuasive; rationales are clearly stated. Research is adequate, timely, and relevant, and addresses all of the issues stated in the assignment criteria.
