(b) In your answer, also provide ONE limitation of the theory that you selected.

3. Which ONE assessment phase from PRECEDE-PROCEED would provide useful information to your small group's planned intervention? Define the phase. What does it do?

(You do not heed to know the number of the phase-just describe what kinds of information are collected in this phase of assessment.) How would you use this information in the development of your intervention?

4. Select ONE assessment phase from PRECEDE-PROCEED. Define the phase. What types of information would you gather under that phase? Explain how you would apply TWO different theories(any level, your choice) to this phase in order to collect the information.

5. Under which phase of PRECEDE-PROCEED do we examine predisposing enabling, and reinforcing factors? (You do not need to know the number of the phase-just describe what it does.)

(b) For each of these three levels of evaluation, what is ONE limitation that you might encounter in evaluating a program.

7. Describe the four-stage planning approach for communication campaigns. For each stage, describe an example of how you would apply the approach to a campaign topic (your choice: ). Explain how you would use ONE construct from any community-level theory (your choice: DOI, CO/CB, or PRECEDE-PROCEED) to support the campaign.

8. Your employer has asked you to work with a community to understand their existing social systems, their health challenges, and their desire for change.

Use any community (your choice of community, your choice of health-related problem) to define and describe any THREE of the five constructs of Community Organization / Community Building Theory that would help you understand the community and support them in achieving their desired goals.

9. Suppose that you wanted to encourage more NAU students to select healthier foods. First, define the community of students on whom you would like to focus.

Explain one innovation that you would like to implement to achieve your goal of healthier eating. Define and apply the five key processes of Diffusion of Innovations Theory to get this innovation accepted by NAU students.

10. Consider the properties of an innovation that would have an impact on the likelihood of a behavior change among NAU students (your choice of innovation, your choice of health behavior).

Which TWO of these properties do you think would be most effective? WHY? Explain how a health educator might apply them as you help diffuse this idea through the NAU student population.
