-Hopelessly unrestorable , retaining the tooth

-where expensive procedures with an unknown prognosis

The last decision if it's questionable and I can't do conventional or nonsurgical retreatment --->Do extraction
if the pt. complain and I can't reach the proper treatment or if the prognosis questionable or tooth has a problem ---> Do extraction
we should take decision with the pt. by informed consent
