The purpose of this project is to:
? Describe the process of creating a new database.
? Design a simple Access database for your chosen investigation.
? describe that other data such as laboratory diagnostics can be used within the database.

You may use Google Forms rather than Microsoft Access for this project. Based on your experiences with the Unit 5 Project, design another Google Form with at least 10 problems (health-based). You should have the first problem ask for a "Full Name" with the other 9 problems asking about specific health related responses. This is a possible problemnaire you would send out to a population.

Add your instructor as a collaborator and  sthe following:
1. Link to database (i.e., the spreadsheet with the responses)
2. Link to Live Form
3. An explanation on the ways you could manipulate this data (e.g., t-tests, filters, GIS, etc.)
