The purpose of this paper is to assess a community you are working in and to identify and assess trends. A community report card format will enable you to determine how well the community is doing on selected indicators. In practice the C/PHN would work with community agencies to identify indicators and analyze trends to determine if goals are being met. In this exercise, you will choose to analyze data from the community your clinical agency location. If the agency focuses on the population in a city/town, then use city/town data. If the agency focuses on the county population, then use county data. If the agency focuses on a small community (population < 30,000), then you may need to use country data for some indicators, and you will need to describe this in the analysis. The paper will follow the following template. You will use data from three data points (e.g. 2000, 2005, 2010) under those indicators identified with an asterisk (*). All data should be reported as rates. You can list the data in a table, and the analysis should be directly under the data table when required. References for data should be cited in text and in a reference page following APA format.

Geographic Boundaries
Here you will describe the community and where it is located.

Race and Ethnicity*
Here you will provide the information and also analyze the trends from the three data points

Family Type*
You will include the types of families identified in the census data and analyze the trends from the three data points.

Housing*
Include median costs, description of number of bedrooms and bathrooms, rented , and owner occupied.
Also include an analysis of trends.

Education*
Include the educational attainment (last grade completed from grade school through professional degree), schools in the area according to type (public, private, grade schools, middle schools, high schools, vocational schools, etc). Include an analysis of trends.

Employment*
Include unemployment rate and occupation. Analyze the trends.

Poverty Status *
Include rate of poverty for the population; rate of population subgroups: children and female head of household, and median income. Analyze trends.

Health Status*
Include live birth rate, rate of teen births, infant mortality, causes of death for children 0-5 (rate), causes of death for adults (rate), and crime rates.

Health Care Services*
Include number of provides (physicians, nurses, nurse practitioners, etc), number of hospitals, number of hospital beds. Analyze the trends.
Churches*
Include the number of people who report attending churches/religious gatherings. Analyze the trends.

At the end of the report card, provide a one-two paragraph summary of the overall trends of your selected community. Assign a grade for the community based on trends, and describe your rationale.
