Professionals who screen clients for case management needs use a variety of tools. They have to collect information about the client that helps them create a holistic idea of the client's needs. Once, they have collected information about the client, they analyze it to create a strengths based plan that matches the client's needs with appropriate services. For this Discussion, go to the website below and choose two different assessment tools. Summarize the purpose of each tool and explain why a professional would choose to use it.

Once you have written your summary, answer the following questions.

• Complete one of the assessment tools that you selected for a hypothetical client and post a short summary of the client's information (demographics and presenting problem). Then, add the answers that the client might have given on the assessment tool.

• Write a short analysis of the client's responses and identify two or three needs that the client has that will require case management services.

• Compare the assessment that you used in your example to a similar one (SBIRT to AUDIT or CAGE AID).
