Question: Objective: The aim of this assignment is to teach you how to write a treatment plan. In a variety of work capacities and work environments, Human Service Professionals (HSPs) are tasked with writing treatment plans for their clients. These treatment are useful in many ways; they help you, as the HSP, to organize your work with your client, they are useful in the coordination of care efforts, and are sometimes required for the continuation of care from various third-party payment providers (insurance companies). The development of a treatment plan is also aligned with Specification 5 and Standard 15 of the Council for Standards in Human Service Education (CSHSE) Curriculum Standards: 5. Planning and Evaluation: Context: A major component of the human services profession involves the assessment of client needs and client groups and the planning of programs and interventions that will assist clients and client groups in promoting optimal functioning, growth, and goal attainment. At regular intervals, the outcomes must be evaluated and necessary adjustments made to the plan both at an individual client and program level.

Standard 15: The curriculum shall provide knowledge and skill development in systematic analysis of services needs; planning appropriate strategies and implementation; and evaluation of outcomes.

Your Role: You are a human service practitioner named Nam Tran and you are assigned to work with a client from the case study attached .

Your client is Buddy, age 16, but you will also work with the entire family at times. You are a member of a treatment team working with this client. This team consists of all professionals working with the client (i.e., medical providers and school and community agencies). You are asked to attend a coordination of care meeting with the client's treatment team and you are required to write and present a treatment plan report for the client.

Your Audience: Remember when you are writing this report to think of your audience (your treatment team). Thus, it is extremely important that your writing is professional and concise. Only include information that is needed in this report.

The Report: Your report must contain the following elements:

1. Brief description of the Client. (This section should include a brief description of who the client is. This section should be 4 or 5 sentences long and include information such as age, gender, ethnicity, current living situation, school status, recent treatment experience, attitude towards treatment, physical presentation, etc.)

2. Client and Family Strengths: (This section should include the client and families strengths in areas such as interests, abilities, activities, personality, affect, etc).

3. Needs, concerns, and problems: (In this section, include why the client was referred for services and what the identified problem/issue is. This section can also include the child and family's perception of the problem).

4. Statement of goals and objectives: (Using MAPS goals and objectives, this area will include 2-3 goals and related objectives).

5. Method: (This section will include descriptive statements of treatment that involves who, what, where, and how often the client will receive services. The modalities used in treatment should also be included (i.e., individual, family, or other support services).

6. Target date: In this section you can include an approximate target date for completion of goals and for eventually tapering off or discontinuing services.

7. APPENDIX: In this section, you will include:

a. APPENDIX A: Include an explanation of the helping skills that you used with this client and why these skills were used. Since you have not worked directly with this client, you are of course to use your "imagination" on this paper. Audience: Your supervisor would like you to write provide him/her with a one page report of the helping skills you are using with this client

b. APPENDIX B: Include a one-page reflection about what writing this treatment plan was like for you. Audience: Your professor.
