Assessment: Case Management Meeting and Knowledge Questions

This assessment is designed to allow you to demonstrate your knowledge and skills in all aspects of case management. Part 1 requires you to prepare for and participate in a simulated case management meeting. Part 2 requires you to respond to questions regarding case management practices.

You will be assessed on your ability to:

  • Demonstrate critical thinking skills to analyse, evaluate and respond to each of the questions below
  • Apply knowledge of case management approaches (theory), information sharing, planning, goal setting, monitoring and review
  • Apply knowledge of legislation, statutory mandates and organisational policies and procedures
  • Demonstrate literacy skills to communicate complex ideas
  • Show understanding of group dynamics and competing priorities.

Part 1- As a result of your interview with Kevin you have very real concerns for all involved and need to conduct a case management meeting.

You are to conduct a role play simulating a case management meeting. You are to include all relevant stakeholders (minimum of two other professional persons not including the client/s).

As the case manager you are to:

  • Write an agenda for the meeting outlining each of your immediate concerns as identified in your complex assessment.
  • Write a response for each person in the meeting which will address each agenda item.
  • Each stakeholder must have a conflicting priority on at least one item, e.g. which service should take priority.
  • Meeting must conclude with a case plan that addresses the immediate issues.

On approval of the agenda and stakeholder priorities arrange for classmates or friends to participate as the 'stakeholders'. Brief the 'stakeholders' on the agenda and their individual stakeholder focus prior to conducting the meeting.

Part 2 - Respond to each of the following questions.

Q1: What steps would you take to arrange relevant stakeholders prior to the meeting?

Q2: Who would you invite to attend?

Q3: How would you record the outcomes of the meeting?

Q4: Once a clear case plan has been identified, how do you involve clients in decision- making processes?

Q5: Outline what you would do at his next appointment.

Q6: How would you manage competing opinions and agendas during this case management meeting?

Q7: How does the Privacy Act impact on sharing client information in respect to external referrals?

Q8: Research one of the following and explain how the act you choose is relevant in your role as a case worker  (minimum 100 words).
