Database Initial Study : This is the phase where the decision is made of what the goals are when it comes to the database.If a database is designed and there is no idea what the concept of it will be. It will just be confusing. Ex. A database for sales.

Database Design: This is the phase where the physical design of the database is created. There needs to be a layout (design) for the database and what are the components for the design. Ex. hardware, software, procedures, data access language, etc.

Implementation and Loading: This is the phase the database is installed and data is loaded. If the design specifications aren't implemented.

Testing and Evaluation: This is the phase where the database is tested. If the database isn't tested there can be problems with connections and usage. Ex. loading the data, setting up the users and security.

Operation: This the phase where the information is produced and how it flows. If the database isn't operational then the users won't be able to use it. Ex.

Maintenance and Evolution: This is the phase where changes are introduced. If the other steps haven't been fixed then the database won't run correctly. Ex. Boosting performance, additional user requirements are met.

The first possible database failure that may occur once a database is placed into operation is that of a network. There can be a lost connection between the client and database server. The second possible database failure that may occur once a database is placed into operation is when it comes to user process. This is when a single database connection fails.
