Organisation has tasked to decrease rate of a particular kind of incident or particular type of high risk hazards which is commonly occurring. Task needs to address the following with respect to particular incident in accordance with organisational procedures.

1. Choose incident or particular kind of high risk hazards that is commonly occurring at organisation. This must be supported by analysis from incident trend or hazard trend.

2. Create a policy which is specific for decreasing the hazard/incident identified in problem 1.

3. Establish objectives and programmes as given below:

a. at least one objective
b. at least one key performance indicators (KPI)
c. at least one target.
d. at least 3 programmes with specific actions to be taken, responsible persons, resources required and timeline

4. Create strategy for stakeholder communication, participation and consultation.
