Your Uncle has just discovered that his hospital and primary care physician practice are part of a patient-centered medical home (PCMH). He has no idea what that is and wonders if that means he will now be eligible for home visits. He also got a notice that he needs to enroll and go to his patient portal if he wants to get his test results from a recent doctor visit.

1. Explain the concept of patient-centered medical home (PCMH) to your Uncle and how it will improve the health care he receives.
2. Evaluate how information technology will help in the development of PCMH
3. Create a training presentation for your Uncle or other people on how to use the patient portal to:
   1. Sign on and off
   2. Retrieve or view test results
   3. Make an appointment, or check the day and time of your next appointment
   4. Ask a question about your treatment
   5. Review the notes on an office visit

The presentation should be at least 5 slides long, and if you do not add audio to each slide you need to add notes containing what you would say when presenting each slide to your Uncle or someone else.
