You will find document attached one is a report in that topic we want the same topics in our report, and in the other document is an excel sheet that shows results of answers from seven people answered a questionnaire. So these two is related because in the report you have to talk about the answers you have got as it shows in the example report. You don't have to write a new questionnaire. You can use it and write different answers.
