To achieve M1, you must make effective judgment when you explain in details the philosophies of working in partnership making reference to the case study and your own work experience where possible (A.C 1.1).

M1 also achieved by not only explaining in detail how differences in working practices and policies affect collaborative working and but also the different policies put in place to ensure effective collaboration among care providers making to the given case study (A.C 2.3).
AND The student had submitted the work on time

A range of sources of information have been used

To achieve M2 you must use appropriate range of resource materials such as Books, Journals and internet and cite all references and sources of material used both in-text citation and reference page consistently with the assignment using the Harvard referencing style.

To achieve the M3, you must ensure that the appropriate structure, approach and academic language has been used consistently when applying the given case study in your work.

Your work must be concise and succinct, developing your academic arguments in an ordered way

ASSIGNMENT

Working in partnership is a key element of practice within health and social care. The concepts of collaborative working, consultation and joint ways of working are essential for effective service provision. It requires different health care providers to come together to share their knowledge and ideas on a particular areas for improvement. Those working in the health and social care sector are duty-bound to recognise the significance of promoting autonomy with individuals. They are expected to be attentive and work effectively in their own roles and responsibilities and they relate to others within the sector in an effective way to drive up the standard of care. Partnership working can help with bringing together skills, sharing information, achieving continuity of care, apportioning responsibility and accountability, co-ordinating resource planning and focusing on the needs of service users. In some cases negative results had been achieved which can be linked to various factors such as differences in professional perspective, lack of autonomy and non-joint working.

In this assignment you are expected to convey your knowledge, understanding of collaborative working and good practice as well as make relevant recommendations for improvement. You are to produce a REPORT titled:

Improving Working in Partnership across the Health and Social Care Sector: The Case study of Mr Ian.

Your report should be in three sections meeting all the assessment requirements. You must follow the report format available on stpmoodle.net. You are required to relate your answers to MR IAN SERIOUS CASE REVIEW. You should ensure you have studied the case review and use it both in and out of classroom. You must relate them to the section/assessment criteria and analyse them using different sources of information. The report must be submitted as one document.

Please note: The MR IAN (Mr I) SERIOUS CASE REVIEW and the summary of the case study are uploaded in the ‘assignment pack folder' on stpmoodle.net. You must research the internet and use other relevant materials to support your work.

Suggested word count this report is 4000 words plus or minus 10% including Executive summary, Introduction, conclusion.
