*Appropriate sources are the student's teacher, work samples, reports and the student's family
This template then forms the framework for the written report. The framework is a thinking and planning tool only and the report does not need to include it.
Students should accompany their report with...
1. a lesson plan that shows:
·What the learning objective was, based on the diagnostic task.
·Exactly what the child was asked to do
·How formative assessment was used throughout the lesson
·How the task was summatively assessed
·An evaluation of the lesson leading to a recommendation.
2. evidence of the assessment process being implemented
Students will be provided with a template on which to complete the lesson plan, which must be included in the report and not in the appendix to the report. The lesson plan can be based on a child not necessarily in a school-based setting. In fact, it is advisable that the lesson be conducted in a home-based setting if possible. Note that if access to a child within a school setting is needed at any time, students will need a Working with Children Check. Consent from parents and caregivers will be required when working with a child. The consent form to be filled in is available below. This must be attached in the one assignment document.
