Step One: Your task is to critique the screen design provided using good design principles for data entry and data quality. Evaluate the screen design and content to identify ways to improve data quality, including the comprehensiveness and appropriateness of the data fields. Don't forget to think about the field names! Are they appropriate? Should there be more, should there be less, etc.? Your screen design evaluation must be a minimum of 1 page in length.

Step Two: Once you have determined the appropriate screen contents. Create a paper form using Excel that can be used to collect the same data at the facilities which are not currently using the EHR. Be sure to format the cells on the excel spreadsheet to develop a form. Make sure that your form fits onto one page.
Step Three: Additionally, in a Word document, prepare a table which compares the steps and elements of form design to the steps and elements of screen design. Include at least 4 steps/elements for each form and screen design. Write a one page APA summary explaining how the forms/screen design affects the electronic or imaging technology for data/record storage. Also explain how the forms/screen design relates to data storage.
Step Four: Submit for grading the screen design critique, the Excel form, and the comparison table with summary. Be sure to include appropriate APA style include a minimum of two references.
