  • Identify and decribe all the programs run by these two departments that potentially are related to long-term care and thus should be involved in the integration effort.
  • Describe how the services of these departments are currently integrated with each other and with any other related state departments.
  • Choose one program offered by one of the departments and explain how it could be improved to better meet the needs of the continuum (for example: choose Medicare Home Health Care and describe how adding payment for custodial long-term care in the home might benefit clients, providers, etc.).

  • Once on the website, select Veterans Health Administration.
  • From the map, locate Veterans Health Administration for your region of the country.
  • Then, explore the facilities located in your region.
  • What services are available to veterans? For example, a search focusing on Colorado revealed that there are Vet Centers in Boulder, Colorado Springs, Ft.Collins, Pueblo, and 2 in Denver.
  • In addition, the VA Eastern Colorado Health care System is located in Denver and provides an extensive list of all services provide (e.g., respite, hospice, geriatric rehabilitation, etc.).

From your search and possible visit, what Veterans Health Administration site is available in your area?
What services are available to veterans?
What services are not available and how far would they need to travel?
How can the lack of service affect veterans and families? Give examples.
