You are an analyst for a 500-bed teaching hospital. This week you review a report, which indicates that the hospital infection rate for the last twelve months is double what it was one year ago. The Medical Director, to whom you report, has asked you to recommend a process and plan to get to the bottom of this alarming statistic. Obviously, something has gone wrong and needs to be corrected before an error leads to a wrongful death. How would you approach this problem and what would your process and plan consist of?
