As you have read this week, the quality of data in health care has an impact on its usability to improve patient care. 

This week you are asked to do some research and thinking about the readings you have been assigned.

In an essay not to exceed four paragraphs, please provide your thoughts on the following scenario: 

Many health insurance plans now collect data from the  EHRs. These data are used to characterize for " pre" conditions (such as cardiovascular disease, diabetes, and metabolic syndrome). Once data is gathered and health plan members are characterized in respect to these diseases, this information can be passed back to the member, as well as to the member's primary care physician (PCP). The idea is that the PCP will be able to communicate with the member about the predisposition to these conditions and possible interventions.

At the same time, this data may be used to predict cost to the health plan. As a result, the quality of data coming in from the EMR has the potential to help and possible damage many subsequent processes decisions. 

Be sure you address all three issues in the questions below about this scenario:

• What should a the health plan and the PCPs do to deal with the quality of data? (both the original EHR data and the information created by the health plan about the member's disposition to diseases) 
• How does one make an environment that promotes data quality?
• What are some of the problems associated with having poor-quality data?
