Overview: Before beginning the assignment, you should be familiar with the three public health intervention levels. The following case (Case Attached)follows a localized outbreak of hepatitis A and the actions taken by local citizens, governmental organizations, and a coalition of hospitals. As you read, identify specific parties and their actions/interventions to address the crisis. Using the table below, identify the following:

• What: the action/intervention taken

• Who: the organization or person undertaking the action/intervention

• Whether the action/intervention was primary, secondary, or tertiary.

Identify at least three interventions from the case above, using no more than 10 or so words (without an explanation). For each instance, identify the action, the person or organization performing the action, and the type of intervention it is. There areeight separate interventions identified, and two are completed as examples (numbers 4 and 8). The order of your entries does not matter.

• Primary interventions involve activities aimed at reducing factors that lead to health problems.

• Secondary interventions involve early detection of and intervention in the potential development of a public health problem.

• Tertiary intervention is focused on treatment of an existing health problem to lessen its effects and prevent further deterioration and recurrence.

Specifically, the following critical elements must be addressed:

A. Analyze at least two published public health interventions used to address the issue, describing both the interventions and the theoretical basis of each. In other words, what were the intervention strategies used to address this problem and what was the rationale (the thinking behind the strategy, why the strategy works) for those actions, according to your resource?

B. Explain how the selected interventions reflect primary, secondary, and/or tertiary intervention strategies for public health prevention.

C. Explain how the interventions address and/or mitigate the social determinants of the issue (health and healthcare disparities associated with each). Remember that social determinants are the conditions in which people are born, grow, live, work, and age.

III. The Interdisciplinary Team

A. Based on the case study, ideally who should serve on the interdisciplinary team tasked with developing the client's care plan? Why?

B. Analyze the role of each member of the interdisciplinary team in relation to the following:

1. Improving patient care. For instance, you might consider the role of a team member who would have day-to-day interaction with the client throughout the client's hospital stay.

2. Improving patient outcomes. For instance, you might consider the role of a team member who would be responsible for developing cost-effective outcomes.

C. What strategies should be implemented by the team to ensure the patient understands the terms and outcomes of the post-discharge plan?
