Making changes in our behavior requires consistent monitoring of our progress. As you review your journal, identify areas of strengths and improvement as well as factors that influence your ability to change (i.e. social support). For the purpose of this course you will be asked to submit your responses to a series of questions at different times throughout the semester. Be honest in your assessment of your progress toward reaching your goal outlined in the Behavior Change Contract. This is your personal journey and the information you share will be kept confidential and reviewed only by the instructor.

Directions: Respond to the following questions.

1. Select one health behavior(s) you are trying to change along with the pros (benefits) and cons (costs) of making this change. Identify which of those benefits and costs are most important to you. You may use the table below to respond to this question.  What behavior are you interested in changing at this time?

List the pros (benefits) and cons (costs) of making this change. On a scale of 1 (not important) to 10 (extremely important), identify the importance of this change to your life.

2. What strategies are you currently using or plan to use to monitor your progress?

3. What factors influence your ability to make this health change (i.e. level of social support, access to resources, affordability, etc.) ?

4. Describe challenges you anticipate in your journey toward improving your health. Include an explanation of how you plan to overcome those challenges.

5. Describe how you are feeling about your ability to make this change.
