Formulate a survey about setting minimum productivity standards for an HIM Department. Design a set of questions that an HIM manager would answer to set minimum staff performance standards. Parameters: Your submission can take the form of a letter to HIM Managers. You should include an introduction that explains to the HIM Manager why you are creating this survey and how the results will be used. Your survey should include a minimum of ten questions. Each question should have a brief explanation of why the information is being asked, why each question is important in setting productivity standards.
