Exercise 4.1 - Read the first part of chapter 6 in the text and Readings 4.1 and 4.2. Both Whittaker and Reason emphasise the importance of 'designing in' safety through redundancy and mitigation rather than over relying on probability calculations.

Question: You are a senior government official required to approve the set up of a hazardous chemical plant. A probabilistic hazard analysis indicates that the plant design fulfils the Individual and Societal risk criteria set by the government. Taking account of Readings 4.1 and 4.2, state in a letter to the company of not more than 1,000 words, what extra precautions would you require before approval is granted.

Exercise 4.2 - Read the second half of Chapter 6 in the text and Reading 4.3.

Question: You are the same government official as noted in Exercise 4.1. You are required to write a report to your Minister outlining the probable public reaction to the proposal and making brief recommendations for the public communication and liaison campaign (not more than 1,000 words).

Exercise 6.3 - Read Green (1994) (Reading 6.3) and Callahan (1988) (Reading 6.4). Apply the NORM method to the case study on whistleblowing. Clearly state the Proposed Moral Rule and justify it.
