Behavior change project instructions

So, how did it go? Complete an evaluation on how your project went over the course of the four weeks by either preparing a 6 page evaluation to answer the behavior change project evaluation questions. Don't forget to include any "evidence" you have collected to show your progress.

Content:

- Introduction to behavior change health topic area (1 page)

o Explain why it is desirable to change this behavior.

o Statistics related to health topic area are referenced.

o Describe how your behavior was prior to the start of the project

o Describe your overall goal.

- Body paragraphs (can divide by Week #):

o Each question below was clearly answered for each Week (1-4)

o Describe your target goal (SMART objective) for the week

o Provide a summary of the progress you made toward your goal that week.

o Describe your top 2 motivating factors & explain how they helped you make progress for the week.

o Describe your top 2 barriers & explain how you overcame those obstacles for the week

o Describe your resources you used & explain how you used them to make progress toward your goal that week.

- Conclusion

o Explain how your experience was working towards your goal over the 4 weeks.

o Describe what you could do to continue to work toward your goal. What might be your next goal?

o Describe what you could have done differently to work toward your goal

10 Paper Format:

- The paper flowed smoothly.

- Typed, double-space, 1 inch margins, Times New Roman and 12 font size.

- 4-5 pages NOT including cover page and reference page in APA Format.

o Minimum of 3 references used (May use class textbook as 1)

- Well written with proper spelling and grammar.

- Appendices: Included "evidence" of working/meeting their goal. (e.g. graph with the number of daily steps)
