A patient is being discharged from an acute care facility with a prescription for a new medication. Knowing the medication has several electrolyte imbalances as side effects, the RN incorporates this information into the discharge plan.

• Develop three teaching points with rationales for your initial posting.
• Include special dietary instructions when appropriate.
• Be sure to include adverse reactions that the patient needs to report to the primary health care provider.
