Assignment
This assignment is about designing and describing a form to gather data that will be entered into a system.

Background
The background of the client is given in file Assignment 1 Document.pdf on the Moodle site. There are 6 forms noted towards the end of this document that support the case. They are numbered from 1 to 6. Your client has expressed concerns that they are often completed incorrectly (or not at all) and that the staff who need to complete them do not understand all of the prompts (or worse still believe that they do, but complete the forms incorrectly)

Scenario
Your client has asked you to examine their business analysis notes, i.e. the aforementioned PDF file,select ONE form out of the six, and redesign it to match their business needs.

Tasks
1. Analyse the intended users by creating at least three Personas, at least six Relationship Questions for your personas, and estimated answers to these questions by your personas. The personas should cover the most typical user groups (form fillers and/or form readers depending on the context of the selected form).If your personas cover more than one user group, set up at least four relationship questions for each user group. The relationship questions may be adjusted to different user groups to accommodate their different concerns and focuses.

2. Create a Question Protocol to list the questions you are to use on the new version of the selected form, who need to know the answers to these questions, and the purposes of knowing them.

3. Create a new version of the selected form using your questions in the question protocol. Adjust the layout, wording, etc., to professionally present the form. The new version can be created using Word or Visio, or using some online form creation tools (e.g. formsite.com, typeform.com, etc.). If you create online forms, please submit the URL of the form and the snapshots of every page of your form.

All answers should be specific to the selected form and the scenario of Canberra Disability Services.

Standard

1. Personas and relationship questions-Personas are relevant to the scenario with appropriate characteristics according to the action of filling/reading the selected form.Relationship questions cover most aspects regarding the personas' reactions to form filling.

2. Question protocol -Clear identification of related stakeholders that need what information for what purpose. The list of questions is relevant to the form and can grab most needed information through the form.

3. New form - Major ill-designs of the original form are fixed. Questions are specific, arranged in a logical way, and provided with appropriate form controls. Good language use, clear and useful instructions.Neat and tidy layout. Good consideration on form usability and accessibility.

4. Professional Presentation - Components present and mostly well integrated. Generally clear writing style, with very few misspellings or grammatical errors. Good effort made but not outstanding
