Read the article titled "IT Project Failure Rates: Facts and Reasons" located in the online course shell, or click here to view it on the web. Then, respond to the following in not more than 250 words:

Identify an IT system development failure that occurred within the last 2 years. Do not repeat an example from earlier in the course or one that has been posted by another student.

Analyze which lapses in the system development life cycle (SDLC) led to the failure.

How might the organization have avoided these pitfalls?

Discuss the Plan-Do-Check-Act risk management process model. Point out the benefits gained by using such a model, and provide real life examples.
