Assignment - Nursing Care Plan

Task 1: Setting the scene

a. Select one of the client scenarios available on the BN605 Moodle site.

b. Identify the scenario you selected.

c. Identify the health care service to which the client has been admitted.

d. Briefly summarise the main reason(s) why the client was admitted to that service.

e. In one paragraph, discuss your chosen 5 nursing diagnoses and the reason (rationale) you have prioritised these for the selected patient. The discussion must include reference to evidence-based practice you have used to determine your priorities.

Task 2: Short-term care planning

a. Download the nursing care-plan template from the BN605 Moodle site.

b. Identify five key nursing diagnoses for planning care in the first twenty-four hours after the client's admission to the service.

c. Complete the nursing diagnosis, and the client centred outcomes (plan), and nursing interventions using the electronic template supplied.

d. There is no word limit for this task, however a minimum of six interventions per diagnosis is expected.

Task 3: Lifespan considerations

a. Identify and discuss the relevant lifespan considerations specific to this client that must be factored in to their care-plan.

Task 4: Discharge planning

a. Discharge planning should be commenced at the time of admission to the service. Clearly identify and discuss three aspects of your client's care that need to be addressed in their discharge plan.

Task 5: Information evaluation

a. Briefly evaluate whether the information available to you in the scenario was sufficiently detailed to allow for the development of an appropriate care-plan, and what other information, if any, do you also require.
