The purpose of this Assessment 1 is to collect and research current information on hazards associated with a work task.

Task 1.1

Select a work task associated with a building and construction workplace.

a) Collect Workplace health and safety (WHS) information and data relevant to this work task from both external sources including legislation, and internal sources such as examples of information sources and data found in the workplace.

In doing this, seek input from stakeholders, key personnel and WHS specialists.

b) Review these workplace sources of information and data that have been accessed above in Task 1.1 and use these to assist you to identify hazards associated with the work task selected.

Collate your results and present your findings as a report or database.

Ensure that your information is current in respect of workplace issues.

Assessment 2

Develop a risk register and prepare a report.

Task 2.1

Identify three hazards associated with the work task from Assessment 1.

Resources needed to complete Assessment 1

You may need access to the following resources to complete this assessment:
• access to actual workplaces and workplace documentation
• reports from other parties consulted in identifying hazards and controlling risks
• access to relevant legislation, standards and guidelines.

The information in these resources you need to access could include legislation, codes of practice, guidance materials, industry sources, databases, WHS specialists.

Task 2.2

Develop a risk register using information sourced from a document such as an existing Job Safety Analysis or similar tool that identifies:

a) the risk factors ie. the likelihood of the hazard causing injury or illness (almost certain, likely, possible, unlikely, rare)

b) the control methods associated with these hazards

c) the contributing factors. This risk register should relate to the hazards identified in Task 2.1.

Task 2.3

Using the current controls in place, prepare a report which includes:

• workplace emergency procedures relevant to the hazards
• issues that may arise from existing control methods
• how the control methods meet legislative requirements
• details of how the risk is reduced as far as is practicable
• a suggested list of alternative control methods which could be used according to the hierarchy of control.

• AS/NZS 4801:2001 Occupational health and safety management systems - Specification with guidance for use
• AS/NZS 4804:2001 Occupational health and safety management systems - General guidelines on principles, systems and supporting techniques

• reports from other parties consulted in identifying hazards and controlling risks
• access to relevant legislation, standards and guidelines.Additional information

Documents could include:

• cause and effect diagrams
• WHS surveys.

Consequences could be:
• death or permanent disability
• serious injury requiring long term recovery
• injury requiring first aid and short recovery
• injuries requiring first aid and no recovery, and minor injury not requiring first aid.

The hierarchy of control is:

• isolation
• engineer/design
• administrative
• PPE.

Assessment 3

This Assessment 3 relates to the following:
• assessing external sources of information and data to identify hazards
• analysing the work environment to identify hazards
• assessing risk associated with a hazard
• controlling risk associated with a hazard
• maintaining hazard identification and risk control processes
• monitoring and review risk management processes.

Task 3.1

You are required to answer each of the following fifteen (15) questions.

1) What were the sources of information you used to identify the hazards in Assessment 2?

2) Who did you consult with as part of the identification process in Assessment 2?

3) What changes to a workplace could impact on the hazards or the hazard identification process? Answer this either in relation to the workplace and hazards identified in Assessment 2, or in a general sense for any construction workplace.

4) When should a hazard identification be conducted?

5) Who should be involved in a risk assessment?

6) Who did you consult with regarding potential hazard control methods in Assessment 2?

7) What legislation or codes of practice did you look at when developing these control methods?

8) How did you prioritise the control options?

9) How could you measure the effectiveness of risk control methods?

10) Using the example of the work task you chose in Assessment 1, give three examples of factors in the working environment which may impact on health and safety of persons in relation to this work task.

11) Give four examples of agents that could cause injury or illness. These agents could be:
• chemical
• physical
• biological
• ergonomic.

12) Who were the stakeholders you consulted with in Assessments 1 and 2?

13) Who are the personnel who should be involved in the risk management process?

14) Why is it not advisable to use generic hazard and risk checklists?

15) How could the language, literacy and cultural profile of a workforce affect occupational health and safety?

The purpose of this Assessment 4 is to develop an action plan for the monitoring and reviewing of a risk management process.

Task 4.1

Develop an action plan for the monitoring and reviewing of a risk management process. The plan could be presented as a flow diagram or chart. Details must include:
• frequency
• methods and scope of reviewing process, including causes and patterns of individual incidents/illnesses
• processes for identifying adequacy of control methods
• methods of communicating changes to stakeholders and operational staff
• particular responsibilities and timeframes
• how improvements to the process are to be actioned.

Task 4.1
Prepare a report explaining the implementation of the action plan for the monitoring and reviewing of a risk management process.

This could be prepared as a PowerPoint presentation limited to a maximum of 15 slides, giving an overview of your action plan.

Details must include:
• the rationale behind the action plan
• sources of information used to ensure currency of information
• relevant stakeholders who were consulted during the review process
• situations requiring specialist consultation.
