As a facilities manager you are a part of a care coordination team. Health care coordination teams do the following: advocate, are patient champions, health coaches and mentors. The team assists patients with navigating the health care network and improves patient outcomes. In addition, the team creates meaningful behavior change within the population they manage. Imagine if you were a part of such a team as a facilities manager. In what ways would you assist to create change for the patients you serve?
