Annual Performance

You are working as a Respiratory Care manager for big healthcare system for a full year now. Please complete your annual Performance Review based on the descriptions below;

Annual Performance Review Instructions:

It is the policy of Health system to conduct ongoing Performance Reviews for all employees. The Annual Performance Review is an opportunity for all employees to reflect upon their hard work and accomplishments over the past year.

Guided by our Employee Promise to retain and develop our best talent, we've enhanced our behavioral expectations to show the connection between our refreshed organizational values and our behavioral expectations

All team members are strongly encouraged to complete a self-assessment .There are two sections to the Annual Performance Review:

1.) Goals*

2.) Behavioral Expectations

Complete each section, how well you demonstrated each of the Core and Leadership Behaviors.

Use the "Comments Section" to highlight specific examples of your performance or key accomplishments, as well as any opportunities to develop.

Goals Summary:

(This section will display the overall goal rating (average of all goals combined). This is an additional opportunity to add comments for each goal as well as comments against the overall goal performance. Please note that if no goals are available a 'Not Yet Rated' rating will be displayed.)

( Description example : Every moment matters. Is passionate about anticipating the expressed and unexpressed needs of others by caring for our patients, our communities and each other. Builds strong relationships and delivers patient/customer centric solutions.)

4. Teamwork

(Description example : Trusts and relies on each other to work together to achieve organizational goals. Leverages and embraces diversity to create a feeling of belonging and committed support.)

5. Execution

( Description example :Takes ownership of work, structures job tasks, and handles multiple deadlines by pursuing with passion the highest standard of quality and service. Is respectful of others and celebrates differences.)

6. Organizational Awareness

( Description example :Understands how to overcome obstacles and works through the realities of a large healthcare organization.)

7. Enable Change

( Description example :We're visionaries looking for ways to innovate and positively impact our Healthsystem. Seeks opportunity to champion new processes and ideas. Promotes, responds, and anticipates change to shape our everyday tasks.)

8. Develop Self

(Descriptionexample: We are all unique. Searches for innovation in everything he/she does and embraces challenging assignments. Seeks learning opportunities to enhance performance.)

9. Managerial Courage

(Descriptionexample: Makes the right decisions for the right reasons by acting honestly, professionally, and consistently. Is willing to champion an idea or position against the norm to redefine what it means to work in health care.)

10. Motivating and Inspiring Others

(Description example: Inspires others and strives to understand how others are motivated. Sparks others to achieve high-level results and pursues with passion the highest standard of quality and service.)

11. Strategic Agility

(Description example:Seeks opportunities to gain/share expertise with others to create and promote innovative strategies. Sees the big picture and articulates clearly the vision and path forward for the team and for the Healthsystem.)

12. Developing Others

(Description example: We couldn't do it on our own. Continuously seeks opportunities for team members to succeed and fulfills the desire to make things better. Provided challenging stretch assignments and tasks to enhance team performance.)

13. Behavioral Expectations Summary Instructions

(This section will display the overall rating (Average of all Behaviors combined) and provide the opportunity to add summary comments regarding overall behavior demonstration. Additionally, this section will display a summary of all behaviors, including their individual rating. You may use this section to modify ratings and add or edit comments for each behavior.)

14. Annual Performance Review Summary

(This section will display the overall rating of your Annual Performance Review (average of the Goals and Behavioral Expectations sections). You may use this section to review the overall rating and add summary comments. You also have the ability to review the ratings for the goals and behavioral expectations sections and add or edit summary comments for these sections.)
