Assessment Guidelines:

This brief requires the student to complete a total of 3 tasks. These tasks should promote a range of aspects of care for the older person and should be carried out with standard and quality assurance

Choose 3 from the following list (Any 1 Task is integrated with the Assignment in Care Support-5N0758)

1. Plan and carry out a task that demonstrates your understanding of a client's need to feel at home.

2. Plan and carry out a leisure activity for a client who has physical or cognitive impairment.

3. Plan and carry out a health promotion activity for a client.

4. Plan and carry out a therapeutic intervention with a client.

In addition, the student will complete a written report for each skills demonstration that should include the following areas:

• Introduction- Indicates where the tasks were carried out and brief profile of the client.
• The importance of the theme of each task.
• How you prepared for each task to include choice of activity.
• Equipment required and safety measures.
• How you carried out the activity to include communication during the task.
• A self-reflection on completion of the task to include what went well or areas for improvement.
